Matrix-Wellbeing Terms and Conditions

Treatment:

Your initial treatment session will last approximately 60 minutes and includes:

 a 15 to 20 minute consultation, including a discussion regarding your lifestyle and medical history

(as a member of the Federation of Holistic Therapists (FHT), Matrix-Wellbeing abides by the FHT guidelines which require the medical history of clients)

an up to 45 minute treatment, tailored to your individual needs.

Your follow up treatments will last 30 or 60 minutes and will include:

a 5 minute re-assessment/consultation, and a 25 or 55 minute treatment.

You will only be charged for the length of treatment you received including oils or lotions used.

Treatments will only be conducted if Matrix-Wellbeing deems the treatment safe for the client to receive.

GP/Consultant permission may be required before treatments can be provided, if you are unsure as to whether you may require your GP’s permission please discuss any concerns prior to booking.

Cancellation policy:

A booking is confirmed once Matrix-Wellbeing and the client have both agreed a date, time, duration and location for a treatment.

If the client is more than 15 minutes late for an appointment, Matrix-Wellbeing has the right to cancel the treatment and no refund will be granted.

Please be aware that no additional time will be given for clients arriving late for their appointments and this may result in a shorter session.

Clients are asked to provide at least 24 hours prior notice when cancelling or re-scheduling appointments.

Matrix-Wellbeing has the right to cancel a treatment by giving 24 hours notice

Matrix-Wellbeing has the right to cancel a treatment without notice if the room or environment is unsuitable for the treatment to be provided, or if the therapist is taken ill at short notice, Matrix-Wellbeing will attempt to substitute another therapist.

Matrix-Wellbeing has the right to cancel a treatment without notice if the client is intoxicated or inebriated.

Anyone not attending a booked Matrix-Wellbeing appointment without giving the required notice and not making payment in advance will be invoiced (see Payment below).

In the case of a non-attendee, Matrix-Wellbeing has the right to require payment in advance for any further Matrix-Wellbeing appointments via the vouchers scheme within the Shop.

Matrix-Wellbeing commits to rearranging treatments to a new time and date within three weeks of the cancelled treatment wherever possible.

Vouchers:

Vouchers can be used as payment or partial payment towards any available or advertised treatment.

Vouchers are only valid for 18 months from the purchase date.

Vouchers once fully redeemed are no longer valid.

Please be informed that no card details are ever held on Matrix-Wellbeing’s servers, any data is securely processed and handled by Stripe.

Course of 5:

20% discount is received when block booking 5 treatments.

All 5 Treatments must be redeemed within the 18 month period.

Treatments will not be refunded if they are not redeemed within the 18 month period.

It is the clients responsibility to ensure they redeem all 5 treatments.

Discount codes:

No further discount is applicable to the course of 5 treatments as this is already discounted by 20% or to the Initial + One voucher.

Website Content:

Website content will be authorised and managed at Matrix-Wellbeing’s discretion, including testimonials.

Photographs will only be displayed with the consent of the people in the photograph.

Photograph consent may be obtained by Matrix-Wellbeing verbally, or in writing if the client completes a photo release form.

Photo release forms can be requested by phone or email.

Payment:

Matrix-Wellbeing accepts payment by cash, website vouchers or card. Invoices will be issued to clients who do not pay for, or who cancel treatments at short notice.

Invoices must be paid within 7 days of the issue date.

Please be informed that no card details are ever held on Matrix-Wellbeing’s servers, any data is securely processed and handled by Stripe.

Privacy:

Client treatment details will not be discussed with anyone other than the client, unless the client is under the age of 18 or has a care worker or guardian.

Consultations will only be discussed with the client, unless the client is under the age of 18 or has a care worker or guardian.

Consultation forms and treatment notes will be kept on file according to the data protection act 1998 by Matrix-Wellbeing.

All clients must complete a consultation form prior to receiving a treatment.

By signing the consultation form, clients are giving Matrix-Wellbeing permission to hold records and data about them.

Personal information will not be shared with any third parties.

Young persons under the age of 18 must be accompanied by an adult.

The adult must give permission for the treatment to be performed.

The adult must co-sign the client’s consultation form.

The adult must be present in the room throughout the treatment.

Insurance:

​​Matrix-Wellbeing is a member of FHT and fully insured by Hiscox Insurance as well as Westminster Insurance Ltd.

Inappropriate behaviour:

Matrix-Wellbeing has the right to terminate a treatment immediately if the client demonstrates inappropriate, offensive or sexual behaviour.

Payment will be invoiced for the full treatment price, provided that over 10 minutes has been spent treating the client.
